X9 Designation Shows What FDA Breakthrough Device Status Does and Does Not Mean
The designation brings closer contact with FDA reviewers and prioritized review, but no change in the evidence a device needs to reach the market.

Key points
- X9 said on Oct. 26, 2026, that FDA granted breakthrough device designation to its investigational ultrasound system with power-assisted needle insertion for hemodialysis.
- FDA says designated devices must still meet its standards for safety and effectiveness through a 510(k), De Novo or PMA.
- As of June 30, 2026, FDA had granted 1,320 breakthrough designations and 210 marketing authorizations for designated devices.
X9, Inc., a Wallingford, Connecticut, device company, said on Oct. 26, 2026, that FDA has granted breakthrough device designation to the X9 Ultrasound System with Power-Assisted Needle Insertion, according to the company's release, distributed by GlobeNewswire and read via Via Ritzau. The company said the designation covers imaging and guidance to help identify validated vascular access sites and assist guided needle insertion for hemodialysis cannulation.
The release also states what the designation is not: it "does not constitute FDA clearance or approval" of the system and does not guarantee future marketing authorization. X9 said the system is investigational and not commercially available in the United States.
Breakthrough designations appear often in device news. Here is what they involve.
A program, not a pathway
FDA's Breakthrough Devices Program page describes it as "a voluntary program for certain medical devices and device-led combination products." It does not replace the routes to market. A designated device still needs a 510(k) clearance, a De Novo grant or premarket approval, and FDA says breakthrough devices "must meet the FDA's rigorous standards for device safety and effectiveness" to be authorized.
To qualify, a device has to meet two criteria. First, it must provide more effective treatment or diagnosis of a life-threatening or irreversibly debilitating human disease. Second, it must meet at least one of four conditions: it represents breakthrough technology, no approved or cleared alternatives exist, it offers significant advantages over existing alternatives, or its availability is in patients' best interest.
What designation provides
FDA says designation gives manufacturers "an opportunity to interact with FDA experts" on premarket issues while the device is being developed. Options include sprint discussions, data development plan discussions and requests for clinical protocol agreement. The agency adds: "You will also receive prioritized review on future regulatory submissions for the device."
In practice, a designated company still has to generate the clinical evidence its eventual submission requires. X9 said in the same release that it completed ACCESS-2, a single-center feasibility and safety study that enrolled 10 hemodialysis patients with arteriovenous fistulas. The company reported that the study met its primary safety objective and that no serious device-related adverse events occurred. Those results are company-reported.
Designations versus authorizations
FDA's page shows how many designated devices reach the market. As of June 30, 2026, the device center and the Center for Biologics Evaluation and Research had granted 1,320 breakthrough designations, 1,299 of them by the device center. Over the same period, there were 210 marketing authorizations for designated devices, 205 from the device center and five from the biologics center.
The designation has also entered payment policy discussions. At a CDRH town hall during The MedTech Conference in Boston, RAPS reported, officials described RAPID, a joint FDA-CMS coverage pathway proposed in April, for which only Class III breakthrough devices and Class II De Novo devices in FDA's Total Product Life Cycle Advisory Program would be eligible.
