FDA Clears Bayesian Health AI Software That Flags Sepsis Risk From Health Records

The Johns Hopkins spinout says its sepsis flagging device is the first continuously running AI sepsis monitor to receive 510(k) clearance.

The FDA cleared this device through the 510(k) pathway, which finds it substantially equivalent to a device already on the market. Clearance is not the same as approval.

Key points

  • FDA cleared the Bayesian Health Sepsis Flagging Device under 510(k) K250680 on April 30, 2026; the company announced it on May 12.
  • The software reads electronic health record data and posts a flag such as "sepsis risk high" in the record.
  • Bayesian says the clearance positions it for a Medicare add-on payment decision expected in early August.

Bayesian Health said on May 12 that the U.S. Food and Drug Administration has cleared its AI-based sepsis flagging device through the 510(k) pathway, and the company called it "the first continuous AI sepsis monitor ever to do so" in its announcement, distributed by PR Newswire.

The FDA 510(k) database lists the Bayesian Health Sepsis Flagging Device under K250680, with a decision of substantially equivalent dated April 30, 2026. The submission was received on March 6, 2025, and is classified as a software device to aid in the prediction or diagnosis of sepsis (product code SAK). The entry shows no predetermined change control plan was authorized.

How the software works

According to MedTech Dive, citing the FDA's summary, the device analyses data from electronic health record systems, including the emergency department chief complaint, laboratory measurements, vital signs, procedures and medications. It then places a flag, such as "sepsis risk high," in the health record for clinicians.

The company said the software runs across every hospitalized patient rather than waiting for a clinician to order a test. "Unlike Bayesian's solution, other FDA-cleared sepsis solutions require a clinician to suspect sepsis first," the release said, citing biomarker blood tests and risk scores calculated from those tests.

Bayesian, founded by Johns Hopkins University professor Suchi Saria, received FDA breakthrough device designation for the technology in 2023, MedTech Dive reported. The company said the clearance builds on that designation.

Evidence the company cites

Bayesian pointed to a 2022 peer-reviewed study in Nature Medicine covering 764,707 patient encounters, 17,538 of them with sepsis, across five hospitals. According to the company's summary, the system showed 82% sensitivity, flagged sepsis 5.7 hours earlier, and had 89% provider adoption. The company said sepsis patients were 18% less likely to die in the hospital when clinicians acted on its alerts in time. MedTech Dive noted that the study compared patients whose alerts were confirmed by a clinician within three hours with those whose alerts were not.

"FDA clearance is a critical milestone, and it's also the consequence of years spent validating that this fits into clinician workflows and helps them get ahead of deterioration instead of reacting to it," Saria said in the release.

The company named Cleveland Clinic and University of Rochester Medicine among its health system users, and MedTech Dive added MemorialCare in California. James Morrison, chair of the Cleveland Clinic Enterprise Sepsis Steering Committee, said in the release that "This FDA clearance is an important milestone, validating the standard of safety and performance we demand for our clinicians and the patients we serve."

Bayesian also said the clearance positions it for the Centers for Medicare and Medicaid Services' New Technology Add-on Payment program starting in October 2026, with a final decision expected in early August. The clearance covers the sepsis module only; the company said the same platform also targets respiratory failure and cardiac events.
